Figured it out.
The profile only applies to MassEffect2.exe which is incorrect
The correct file is ME2Game.exe
You can either go to Local Profiles -> Mass Effect 2 and hit Create a new profile based on this one, select Keep game specific options and then click Add on the new profile and select ME2Game.exe
OR
Go to Cloud Profiles and search for Mass Effect 2
You should see Mass Effect 2-Fixed [doskir]
Import profile and play
