In Outer Worlds, within VorpX menu, make sure your vertical HUD scaling is set to 100%. The aiming reticle is NOT centered to the HUD, so if you allow VorpX to re-scale the HUD, it will be shifted up compared to where you are aiming.

I tried getting TOW to perform smoothly but it proved very difficult.
Didn’t want to bring the resolution too low (~2880×1600) and the performance was a bit janky with most settings lowered. I guess I could try again with further enigne.ini config stuff and see if I can get it right.
Do you have performance issues at all? Would you mind sharing your main vorpx and game settings?
